Concentration = Vial mg ÷ BAC Water mL = mg per mL

mg per IU = Concentration ÷ 100

mcg per IU = mg per IU × 1000

Example: A 5mg vial reconstituted with 2mL BAC water = 2.5 mg/mL. Each IU (0.01 mL mark on an insulin syringe) = 0.025 mg = 25 mcg.
